Required Documents
The documents listed below are the minimum necessary to proceed with the application process. If you believe that a required submittal document does not apply to your project, please upload the Document Waiver Form in place of the required document.
- Bill To Form
- Construction Plans including the following:
- Existing and proposed equipment plans for all locations
- Location and dimensions of existing facility components
- Existing and proposed site grade. If no change, include a note reflecting no change.
- Area of ground disturbance for construction activities
- Details
- Visual Screening Methods and Materials: This may include vegetation screening. Include location, number, type and caliper (if applicable) of proposed trees, size of shrubs, or cut sheet for material type and color for any proposed screening.
- 6409a Compliance Letter
- A compliance letter provided by the City of Bellevue Development Services Department attesting compliance with the 6409a criteria.
- Letter of Compliance with FCC RF Standards
- A report and letter prepared and signed by a licensed RF Engineer attesting compliance with FCC RF Standards.
- Owner Authorization
- Basic Site Plan
- In addition to the minimum requirements listed in the Basic Site Plan, site plans for WCFs should indicate area of work on site plan.
